Prelim report (3 killed).

Shortly after takeoff from LCK (Rickenbacker, Columbus OH) on 09-01-08, the pilot of a Convair 580 reported trouble to ATC. When he notified the tower of the need to return to the airport, ATC asked if they needed equipment. The pilot responded, "Negative." Two minutes later, the aircraft crashed in a cornfield 1 mile southwest of LCK's Runway 5L. All 3 on board were killed.

Learjet 60.
Prelim report (4 killed, 2 injured).

On the evening of 09-19-08 a Learjet 60 overran Rwy 11 at CAE (Metro, Columbia SC) during its takeoff roll. Witnesses reported seeing sparks coming from the aircraft and seeing it pass the runway threshold. Portions of the airplane and tire debris were later found along the runway. The Learjet was destroyed by a post-crash fire and the 2 crewmembers and 2 of the 4 passengers were killed. The other 2 passengers were seriously injured.

A Cirrus SR22 crashed near ARV (Minocqua WI) on 09-11-08 as the pilot made a nonprecision approach to ARV in IMC. The flight had departed from MKE (Intl, Milwaukee WI) and was operating on an IFR flightplan. The aircraft wreckage was located about 1/2 mile southwest of the approach end of ARV's Rwy 36.

A Citation 501 was reported missing after departing SDQ (Intl, Santo Domingo, Dominican Republic) for SIG (Fernando Dominicci, San Juan, Puerto Rico) on 08-18-08. The flight was expected to last 1 hr and was last seen on radar approximately 3 miles south of SDQ.

Prelim report (2 killed).

On the morning of 07-15-08, a Hughes 269 was destroyed on impact with the ground after striking a high-power transmission wire. The helicopter crew were conducting a powerline patrol flight in VMC. The wire struck by the helicopter was one of 5 wires crossing perpendicular to and above the wires being surveyed by the crew.

Bombardier CRJ700/Cessna 172.
Prelim report (no injuries).

A runway incursion occurred at ABE (Lehigh Valley, Allentown PA) involving a Bombardier CRJ700 and a Cessna 172. The Cessna pilot completed a landing roll and was instructed to exit Rwy 6 at Taxiway A4. At the same time, the tower controller cleared the CRJ to taxi into position and hold on Rwy 6, and then cleared it for takeoff. The Cessna 172 missed A4 and requested to exit at Taxiway B instead. The CRJ700 crew heard that the Cessna had missed A4 and, when they saw the aircraft still on the runway, they aborted the takeoff and swerved to miss it.

Bell 212.
Prelim report (7 killed) .

Near midnight on the Resilient Maersk oil rig in the Persian Gulf, a hovering Bell 212 backed into a crane, crashed onto the deck of the oil rig and caught fire. After takeoff, the pilot had put the 212 in a hover and executed a 180° pedal turn. Witnesses said the helicopter then drifted backwards into the oil rig's crane and fell to the deck. The United Arab Emirates is investigating the accident.
